Usually, most of them who suffer from back pain may believe that it is due to involvement of bony lumbar spine.  But the fact is different and back pain may relate to many other body parts.  One has to understand that back pain may relate to bony lumbar spine, intervertebral discs of vertebrae, ligaments surrounding spine and discs, spinal cord and nerves, low back muscles, and even internal organs of pelvis and abdomen, and last but not least, the skin which is covering lumbar area.

Electronic Medical Record Management

An Electronic Health Record or Electronic Medical Record Management System is a representation of all a patients’ data that would originally be found in the paper based record. It contains all information ranging from pathology, radiology and clinical information that has been combined and structured in a digital format.

The system is designed to furnish, capture, transcribe and re-present data that accurately capture the state of the patient at all times whenever visited a clinic or hospital. It allows for an entire patient history to be viewed without the need to track down the patient’s previous medical record volume and assists in ensuring data is accurate, appropriate and legible. It reduces the chances of data replication as there is only one modifiable file, which means the file is constantly up to date including the latest personal details when viewed at a later date and eliminates the issue of lost forms or paperwork. Due to all the information being in a single file, it makes it much more effective when extracting medical data for the examination of possible trends and long term changes in the patient.

Using an EMR to read and write a patient's record is not only possible through a workstation but, depending on the type of system and health care settings, may also be possible through mobile devices that are handwriting capable (there are devices called digital writers.)

Electronic Medical Records may include access to Personal Health Records (PHR) which makes individual notes from an EMR readily visible and accessible for consumers.

Some EMR systems automatically monitor clinical events, by analyzing patient data from an electronic health record to predict, detect and potentially prevent adverse events, highlighting allergies etc. This can include discharge/transfer orders, pharmacy orders, radiology results, laboratory results and any other data from ancillary services or provider notes. This system greatly reduced the number of missed critical opportunities.

As Electronic Medical Record Management or EMR is much popular in corporate hospitals and public sectors, as they are utilizing in best possible way to save time, man power, and money, and ultimately minimizing the errors in day-to-day activities.

AYURVEDIC HOSPITALS:

As Allopath hospitals face some challenges, similarly in Ayurvedic field, we do face certain challenges in day to day activities when it comes to practice and patient handling work. As ayurvedic professionals find some difficulties when it comes to delivering health care to the patient.

In order to improve the standards and quality based service, there are certain bodies who come forward for accreditation process. Basically A standard Ayurvedic Hospital does have certain methods in hospital and with the recent standardization methods in the field of Ayurveda, Government bodies and associations started encouraging Ayurvedic Hospitals to improve themselves by certifying them or accreditation methods based up on fulfilling certain criteria. They are AYUSH, NABH (National Accreditation Board for Hospitals & Healthcare Providers), AHAMA etc.

Similar to General Practice, there are certain methods and steps which are taken place in classical way such as when it comes to clinical practice or hospital practice. In Ayurvedic Hospitals, the method of diagnosis and assessment of patient criteria is different from allopath system of medicines.

· When a patient visits Hospital he or she enters Reception.

· From Reception, patient is directed to departments or to consultation chamber.

· Ayurvedic doctors utilize their Ashtavidha Pariksha, or Dasha vidha pareeksha skills, Roga and Rogi Pariksha Skills in classical way, Prakrithi Analysis and so on or according to department.

· From Consultation Patient may be directed to reception or counselor or resident medical officer (RMO) for needful.

· Here, if doctors prescription is only medicines, patient may go to pharmacy and buy medicines and clarifies doubts with doctor or resident medical officer and go away from hospital.

· If doctors are prescribing treatments, patient is directed to visit RMO again who will explain about treatments. When patients are ready for the treatments they will visit hospital for certain time period and take the treatment.

· Another scenario is IP admission. When patient is directed for admission, one should follow all the methods and procedures as prescribed by doctor on daily basis such as treatments, nursing care, Panchakarma treatments etc.


Difficulties and Challenges in Ayurvedic Hospitals:

During the course of examination the challenges we face are:

image Time: If the flow of patients is more, doctors who are consulting may not find time to cover all these assessment criteria. Sometimes ayurvedic doctors may miss their examination criterion.

image Paper Work: Starting From Reception work, OPD Card, Treatment Schedule Preparation, and other paper work which may create confusion among the staff and ultimately we may fail to deliver what we want.

image Man Power: For covering each section of patient handling methods from reception level, front office level, in order to provide maximum customer satisfaction/patient satisfaction we may need man power.

Challenges at different levels:

At Front Office Level: Many a times front office staff may not be in a position to handle the patients when it comes to collect the OP card of the patient or to search the same in the files where they may waste their time that may reflect on patient satisfaction levels.

At the level of IP Facilities: When Doctor consults and put a patient for IP facility, one should closely monitor patient activities and progress in proper passion where we may miss some information to note down or we may face some technical difficulties.

Pharmacy Level: Many of the times, doctors may not know about the stock of medicines which they prescribe to the patients and there may be confusion and rewriting prescription and sometimes doctors may lose their confidence during re-prescribing medicines.

Accounts and Billing Level:

Most of the time accountant may face some confusions in billings when there is no proper coordination with the front office staff regarding patient handling and collecting the charges and issuing bills to the patient.

Research Purposes: Most of the times, doctors do lose the patient’s data when they want to recollect for some research purposes or to follow up the patient for certain reason. When there is no proper paper work done or when there are certain fields missing to be filled regarding the patient’s condition, doctors may lose interest in doing research.

ROLE OF EMR SYSTEM IN AYURVEDIC FIELD:

By considering all above challenges we face in day-to-day operations of ayurvedic hospitals, the EMR system seems to be best possible solution to achieve goals and give maximum patient satisfaction and clinical results.

Ayurvedic Management of Filariasis

          As Filariasis is a chronic debilitating disease, which is caused by filarial parasite ‘Wuchereria boncrofti.’ Clinical manifestations such as fever, recurrent lymphadenitis, lymphangitis, with edema of affected parts like lower limbs, scrotum are commonly affected. In ayurvedic sciences Filariasis is called as Slipada.

After intensive research studies by the government body of India, they evaluated few therapies which show considerable amount of relief.

The following medicines are indicated:

SaptaparnaGhanavati,

Nithyananda Rasa,

Sudarshana churna,

Punarnavasavam or Punarnavadhyaristha or PunarnavadiKashayam or PathyPunarnavadi Kashayam are useful medicines and dosage and course can be planned by physician after consulting.

Central council of Indian medicine introduced Ayush-64, a coded medicine which contains (Saptaparna/ Alstonia scholaris, Kiratatika/Andrographis paniculata, Katuki/ Picrorhiza kurroa, Latakaranja/ Caesalpinia bonduc) is also believed to be effective medicine.

Usage of Haritaki or Terminalia chebula is found effective medicine if used in different modes.

Method 1: Haritaki fried in castor oil and made into powder can be used in the dose of 4gm to 6gm with 200ml of milk or 200ml of lukewarm water.

Method 2: Soak Haritaki in mustard oil for overnight and dry it under sun in day time, after getting dried, make powder and can be consumed with 4gm to 6gm dosage with honey or 7-time filtered cow’s urine 50ml.

            Most of the individuals those who suffer from gastric related complaints like hyperacidity may suffer from the symptoms such as feeling of food coming towards mouth, burning sensation in the chest region and these symptoms aggravates on bending forward, or while lying down, feeling of vomiting sensation, sour throat, cough, change of voice, food regurgitation. Lifestyle Management:

When person suffers from psoriasis, one should understand about the disease that is due to weak immune system and try to avoid aggravating factors such as Stress, Anxiety, and Habits like smoking or alcohol. If person is having any other associated diseases such as diabetes or Hyperlipidaemia, and that should be taken care.

Personal care:

Moisturize: Applying suitable moisturizers will help to keep your skin moist in order to prevent scaly skin condition and itching.

Bathing secret

Bathing is mandatory for Day-to-Day life as it is having significant importance for being healthy. Though, most of the individuals doesn’t aware for what they do bath but there are many scientific theories and ancient values. As a part of health purpose and to maintain hygiene, bathing is recommended and being practiced by most. There are many other theories hidden or which are not focused to present era regarding bathing and its uses.

The present trend is changed a lot for bathing techniques as to maintain lifestyle in health and wellbeing angle, they are Balneotherapy, Hydrotherapy, Naturopathy etc.

Balneotherapy is an advanced therapeutic practice of bathing with classical essence where herbal medicines or minerals are added to cold or hot water and during the processing massaging is done where subject feels relaxed and gains therapeutic gain.

During ancient times many herbs were used for bathing purpose and even hot balls of iron or metals were immersed in water so that it become hot and attains mineral values which in turn helps for health purposes. To gain pleasant mind fragrance materials such as sandal, rose water etc., were added to water.

By understanding present condition, there are modified Balneotherapies or hydro therapies which were practiced world-wide.

The following therapies are compilations from ayurvedic texts where herbs are used for bathing purposes.

Though ancient scholars mentioned many types of bathing, they were significant to that particular times. Examples as per puranas are “Bath by wind” or “Vayavya Sananam” (Bathing), Sarswatha Snanam (As indicated by elders), “Divyasnana” Bathing in rain water in presence of sun light, “Manasa Sanana” and “Parthiva Snanam” bathing with prayers for mental satisfaction.

There are many varieties of bathing depending on indication and in ancient times, they classified into Nithya Snana (daily bath), Naimithika Snana (causal bath) and Kamya Snana (desirable bath).

As per traditional practices, even after menstruation, special mud bath is recommended for auspicious reasons.

There are many other traditional bathing which were mentioned as per festivals, even oil bath is mentioned as mandatory in part of “Dinacharya” or “Daily Activity.”

THERAPEUTIC HERBAL BATHING TECHNIQUES:

Water Bath: There are many different varieties of water bath starting from cold water bath, hot water bath, pond water bath, distilled water bath etc., depending on the condition of climate, person, place etc.

Milk Bath: As milk is having many medicinal and nutritional properties, one can apply milk or milk cream all over the body and do mild massage followed by water bath gives nourishment to skin and brings lustre on skin.

Ghee Bath: Ghee bath is very important and is indicated for lean persons and for those desiring good tonicity of body can apply Cow’s Ghee or Goat’s Ghee all over the body which is very helpful as Ghee is having (Yogavahi property) or (Catalytic agent).

Cow Dung Bath: Cow Dung Bath is having significant values and gives good tonicity of the body and is found effective for obese people.

PanchaGavya SNanam or being bathed with curd, milk, ghee, cow urine and cow dung is an auspicious bath which is having multiple benefits.

Gooseberry or Amla Bath: By collecting Fresh or Dried Gooseberries and making paste with water and on applying over full body (Including hair) and do mild massage followed by water bath is very helpful as it acts as cleansing agent and nourishing agent to the skin which helps to enhances glow of the skin.

Herbal mixture BATH: The medicines which are having fragrance and medicinal values such as Srigandham, Jatamamsi “Nardostachys Jatamansi,” Bael (Aegle marmelos), Kasthuri Manjal or Special turmeric, etc., can be put in powdered form in water and used for bathing.

Herbal leaves Bath: By using leaves such as Neem, Beal, Lotus leaves and jasmine flowers soaked in water can be used for bating which is having soothing properties for body and mind.

Conclusion: By understanding the above concept, one can follow regular bathing practice and herbal bathing wherever and whenever necessary which helps to refresh body and mind.

Botanical name : *Ferula asafoetida Linn *
Sanskrit: Hingu
English : Asafoetida
Hindi : Hing
Malayalam : Kayam, Palkayam, Karikkayam, Perumkayam

**AYURVEDIC PROPERTIES **
Rasa : Katu or pungent.
Guna : Laghu or light for digestion, Snigdha or slimy, Teekshna or active, Sara or sharp.
Virya : Ushna or hot.

**MEDICINAL PROPERTIES **
Useful part of asafoetida is Dried Resin.
Plant pacifies vitiated kapha and Vata.
It is mainly used for relieving pain abdomen or colic, constipation, flatulence, dyspepsia, asthma, hysteria, chronic bronchitis, whooping cough, epilepsy, even in mental diseases due to its unique properties and is used for liver diseases.

Herbs during pregnancy and Important care

Herbs during pregnancy:

It is believed that Ayurvedic medicines or herbal medicines are very helpful for the purpose of conceiving or to support pregnancy. But one should realize that not all herbal medicines are helpful for the same purpose and few herbal medicines or herbs do hamper pregnancy or affects pregnancy because they do have the action of abortive, purgative etc.
Keeping this concept in mind, one has to think well before consuming any ayurvedic medicines while trying to conceive or while in pregnancy. Few herbal medicines should be used at appropriate time to avoid adverse effects of medicine.
Herbal medicines do have the properties on particular system of the body such as gastrointestinal, reproductive system, cardiac system etc and are classified accordingly. Hence, few ayurvedic or herbal medicines are having the properties such as uterine contractors, abortive properties, laxative properties etc. So, few herbs may not be suitable for the women, who are trying to get conceived.

Grossly, in ayurveda, one should keep in a mind that the medicines which do have hot potency, or having scrapping property or **lekhana** property, or purgative property, abortive property, and few Vata medicines, may hamper pregnancy and these kind of medicines should be avoided. Apart from these there are few herbal medicines which assists conception. They are good to consume for assisting pregnancy.

However, other factors such as female related complaints or menstrual related complaints should be analyzed and line of treatment should go accordingly.

Here is the list of medicine which is not suitable for pregnancy, or those who trying to get conceived.

1. Bala (Sida rhombifolia) : Milk decoction can be made and consumed 50 ml per day in first trimester of pregnancy.
2. Lakshmana (Ipomeoa sepiaria): Milk decoction can be made and consumed 50 ml per day in first trimester of pregnancy.
3. Bruhati (Solanum melenginum): Milk decoction can be made and consumed 50 ml per day in first trimester of pregnancy.
4. Desmodium gangeticum (Shaliparni): Can be consumed in second trimester.
5. Amrita (Tinospora cordifolia): It is useful if consumed as decoction to gain immunity power.
6. Tribulus Terrestris: Can be consumed if there is any kind of fluid retention or pitting edema which is common in second trimester.
7. Solanum xanthocarpum (Kantakari): Can be consumed with milk.
8. Hordeum vulgare (yava): Can be used in the form of decoctions.
9. Maerua oblongiflia
10. Blessed Thistle
11. Burdock Root
12. Chamomile
13. Dandelion
14. Kelp
15. Nettle Leaf
16. Red Raspberry Leaf
17. Spirulina
18. Black Haw
The above mentioned herbal medicines can be consumed even the form of combinations.
Examples:

There are many more medicines which can be used as needed basis and few medicines are useful for development of fetus.



The following herbs should not be consumed:
There are many herbs and culinary spices which are very harmful for those who try for conceiving. These herbs may appear in the form of single herbs or in the combinations, hence one should be very careful while using any spices or herbs in cooking or while consuming any ayurvedic medicines.

1. Angelica (Canda): Stimulates suppressed menstruation
2. Aloe Vera: Not to be used during pregnancy as it is having purgative nature in many cases.
3. Comfrey (Symphytum officinale) or Lodhra: May cause liver complaints
4. Dong Quai (Angelica sinensis) (Choraka): Stimulates bleeding.
5. Fenugreek (Trigonella foenum-graecum) Methi: Uterine relaxant.
6. Goldenseal (Hydrastis canadensis) (Shinithasthapana): It is having haemostatic property and may affect developing foetus and even it is not used while nursing.
7. Henbane(Hyoscyamus niger) (Parasika Yavani): Highly toxic.
8. Horsetail (Equisetum arvense): Not good to use in developing foetus.
9. Licorice Root (Glycyrrhiza glabra) (Yashti Madhu): Should be avoided in pregnancy and lactation as it is having adverse affects on estrogenic and progestin balance and can create water retention and even not to be used in elevated blood pressure.
10. Motherwort (Leonurus cardiaca) (Nagadamani): It may lead to uterine contraction and potential miscarriage, and stimulates suppressed menstruation.
11. Mugwort (Artemisia vulgaris): It is strong uterine stimulant, hence proper precaution should be taken.
12. Nutmeg (Myristica fragrans) (Jatipatra): It inhibits prostaglandin production and contains hallucinogens if consumed in large quantities or even causes miscarriage in large doses.
13. Pennyroyal Leaf (Mentha pulegium): Uterine stimulant. Kind of culinary herb where tea infusion is made and may cause miscarriage if consumed.
14. Goat’s Rue (Galega officinalis): Strong expellant.
15. Shepherd's Purse (Capsella bursa-pastoris): Not safe to use in pregnancy. It can be used during haemorrhage after child birth or during childbirth safely.
16. Bear berry (Arctostaphylos uva-ursi): It removes much blood sugar during pregnancy and nursing.
17. Yarrow (Achillea millefolium): It is uterine stimulant.
18. Cascara Sagrada: May cause electrolyte imbalance.
19. Juniper or Juniperus communis: Strong vaso-dialating, diurectic effect.
20. Poke root: a powerful emetic. (causes vomiting)
21. Wild Ginger (Curcuma amada) (Amragandha): Used for pickles and it may cause uterine contractions.
22. Black Cohosh - Uterine stimulant.
23. Blue Cohosh - A stronger uterine stimulant
24. Borage oil - A uterine stimulant - use only during the last few days of pregnancy.

Apart from above mentioned herbs, there are many more herbs or spices, which may harm the process of pregnancy and may lead to complications. Hence one should notice for information before using herbal products.
However, depending up on disease conditions, there may be necessity of using the above herbs for treating reproductive related complaints. One should be cautious while consuming such type of medicines.
Usually, now-a-days, there is a practice of consuming herbal supplements such as Aloevera juice, or some laxatives or purgatives without consulting physician, and that may harm for the persons those who are trying to get conceived.

This implies for yoga and meditation too. So one should take proper guidance from a trainer for contraindications while doing yoga poses.
